Welcome to the Diocese of Palm Beach! As a new 30+ hour per week employee, you are eligible to enroll in all of the benefits offered by the Diocese of Palm Beach to their Lay employees. The enclosed summary gives you a brief description of each benefit offered. Additional information on each benefit and enrollment forms are enclosed in this packet. Our benefits program includes Prudential Short-Term Disability and Optional Term Life. You can apply for coverage for your spouse and children on the Optional Term Life. The cost of the short-term disability and the optional term life are paid for by the employee. Click on the name of the program, underlined above, for detailed information. If you are applying for the Prudential Optional Life Program at other than your initial new hire eligibility period (within 30 days from your date of hire), you must complete the Prudential Evidence of Insurability form. During your call, one of their benefit specialists will explain the benefits and the associated costs to you. They will also be available to answer any questions you may have. You will be able to enroll for the benefits you select over the telephone. You will not have to complete any enrollment forms for short-term disability benefit.
